2nd Annual Holiday Hold Challenge

Need help maintaining your weight throughout the holiday season? On average, Americans gain 8-10 pounds over the holiday season. Let us help you start and continue your health goals. The 2nd annual Holiday Hold Challenge will run from Thanksgiving through New Years.  Participants will weigh in between November 19th and November 26th and weigh out between January 2nd and January 9th.  

Participants who weigh in and weigh out on the same scale and either maintain or lose weight will earn a $25 incentive in their 2015 Healthy Lifestyles Account.

  1. Employee Health - (805 Central Avenue, 1st Floor). No appointment needed. Hours: Monday-Thursday 7:30am-4:30pm, Friday 7:30am-4:00pm
  2. Your Department - see below
  3. Your Personal Physician - arrange on your own
  4. Weight Watchers On-site Meeting: arrange on your own (online WW will not be accepted)

Did You Know? Reminder About the Personal Health Assessment

Entering your biometric numbers into your Healthy Lifestyles account and taking the Personal Health Assessment are two different things. You will NOT receive incentive dollars for your biometric numbers you had measured at a health screening or during your physical unless you complete the Personal Health Assessment (PHA).

PLEASE NOTE: The PHA is a 60-question online survey that should take you 15-20 minutes. It is a $75 Healthy Lifestyles incentive! If you have pending or denied biometrics in your Healthy Lifestyles account, you probably need to take your PHA! If you have any questions about the PHA, please contact Healthy Lifestyles.

Paperwork Deadline is Quickly Approaching! 

At the end of the year, Healthy Lifestyles receives an exorbitant amount of questions regarding paperwork and the online participant accounts.  Please take a look at these questions to see if they can help you or a co-worker keep things straight as we wrap up 2014. 

When is paperwork due?
Paperwork is due to Healthy Lifestyles via fax, mail, email or interoffice mail to Risk Management by midnight on December 15, 2014. Remember, you are required to submit paperwork within 60 days of the incentive taking place, so the end of the year should only be items that took place in the fall!

What if I have an appointment on December 18th? 
Any appointments taking place from December 15th-December 31st would require paperwork to be submitted to Healthy Lifestyles by midnight on December 31st. 

What else should I do?  What if the incentive is already denied?
If you have incentives that are denied, you can still send in paperwork to receive credit.  We deny incentives throughout the year when we have not received proof of the incentive.  You should be regularly checking your account throughout the year to look for these items.  

When it's done, it's done?
Yes, once accounts are closed by Healthy Lifestyles, we no longer have access to add incentives that may be received late or had previously been denied. So please check your accounts regularly! 

Flu Shots – Get One Today!

It is not too late to get a flu shot. Even if you missed the clinic at your department, you can get a FREE flu shot at EHS (located on the 1st floor of Centennial II). Employees, their spouse/equal partners and any dependents over the age of 3 can get a free shot. Call to schedule an appointment at 352-1990.  

Credit accrued towards your Healthy Lifestyles account during the current year does not actually become money earned until January 1 of the following year. At that time it only becomes "earned" by those individuals enrolled in the City's health insurance plan. If an employee’s spouse or equal partner is no longer covered by the City's health insurance plan as of January 1 (for whatever reason) , the employee will not be eligible to collect the credit on their behalf.
